Welcome to Little Cradle Health Care

These Terms of Service ("Terms") govern your use of the services provided by Little Cradle Health Care, operated by Dr. Uzma Nikhath Bano, located at #188, 5th Main Road, KEB Layout, BTM Layout 1st Stage, Bangalore 560029 ("we," "us," or "our").

By accessing our website, booking appointments, or using our medical services, you agree to be bound by these Terms. If you do not agree with any part of these Terms, please do not use our services.

Medical Services

1. Nature of Services

Little Cradle Health Care provides:

• Family medicine and general healthcare consultations
• Obstetric and gynecological services
• Pregnancy care (antenatal, delivery planning, and postnatal)
• Pediatric healthcare
• General medical consultations and treatments
• Health screenings and preventive care

2. Professional Relationship

• A doctor-patient relationship is established only upon physical consultation at our clinic
• Online appointment booking or website usage does not constitute medical advice or treatment
• Dr. Uzma Nikhath Bano is a registered medical practitioner and maintains professional standards as per Indian Medical Council regulations
• All medical decisions are made at the sole professional discretion of the treating physician

3. Consultation Fees

• General consultation fee: ₹500 (subject to change)
• Fees for additional services, tests, or procedures will be communicated separately
• Payment is required at the time of service unless otherwise arranged
• We accept cash, card payments, and digital payment methods
• Fees are non-refundable except in cases of service cancellation by the clinic

Appointments and Scheduling

1. Booking Appointments

• Appointments can be booked via our website, phone, or in-person
• Appointment confirmations are subject to availability
• We reserve the right to reschedule appointments due to unforeseen circumstances

2. Cancellations and No-Shows

• Please provide at least 24 hours notice for appointment cancellations
• Repeated no-shows may result in requirement of advance payment for future bookings
• Emergency situations are always accommodated regardless of appointment status

3. Wait Times

While we strive to maintain scheduled appointment times, medical emergencies and complex cases may cause delays. We appreciate your patience and understanding.

Patient Responsibilities

As a patient, you agree to:

• Provide Accurate Information: Give complete and truthful information about your medical history, symptoms, and current medications
• Follow Medical Advice: Adhere to prescribed treatments and follow-up recommendations
• Inform About Allergies: Disclose all known allergies and adverse reactions to medications
• Respect Clinic Rules: Follow clinic policies, maintain decorum, and treat staff with respect
• Payment Obligations: Pay for services rendered in a timely manner
• Communication: Inform us of any changes in contact information or medical status
• Emergency Protocol: Understand that our clinic is not an emergency facility; life-threatening emergencies should be directed to nearest hospital emergency department

Privacy and Confidentiality

We are committed to protecting your privacy and maintaining the confidentiality of your medical information in accordance with:

• Information Technology Act, 2000

Informed Consent

Before any medical procedure or treatment:

• For minors, consent must be obtained from parent/legal guardian

Limitation of Liability

1. Professional Liability

We maintain professional indemnity insurance as required by law. Our liability is limited to actual damages arising from proven negligence or malpractice, subject to applicable legal limits.

2. Website and Technology

We are not liable for:

• Technical issues, interruptions, or errors in our website or online booking system
• Loss of data due to technical failures
• Unauthorized access to your information despite our security measures
• Actions of third-party service providers

3. Third-Party Services

When we refer you to laboratories, pharmacies, specialists, or hospitals, we are not responsible for their services, billing, or outcomes.

Intellectual Property

All content on our website, including text, images, logos, and design, is the property of Little Cradle Health Care and is protected by copyright laws. You may not:

• Use our clinic name, logo, or branding without authorization

Dispute Resolution

1. Grievance Redressal

If you have any complaints or concerns about our services:

• First, discuss the matter directly with Dr. Uzma Nikhath Bano or clinic staff
• Submit a written complaint to our clinic address
• We will respond to all complaints within 7 working days

2. Mediation

Both parties agree to attempt resolution through good-faith discussion and mediation before pursuing legal action.

3. Jurisdiction

These Terms are governed by the laws of India. Any disputes will be subject to the exclusive jurisdiction of courts in Bangalore, Karnataka.

Medical Records

• You have the right to access your medical records upon written request
• A reasonable fee may be charged for copying and administrative costs
• Records will be provided within 72 hours of request (excluding emergencies)
• We retain medical records as per Indian Medical Council regulations (minimum 5 years)
• Original records remain the property of the clinic; you may receive certified copies

Termination of Services

We reserve the right to refuse or terminate services in cases of:

• Abusive, threatening, or violent behavior toward staff or other patients
• Repeated failure to follow medical advice without valid reason
• Non-payment for services rendered
• Providing false information that compromises care
• Any illegal activity on clinic premises

In such cases, we will provide appropriate notice and assist with transfer of care to another provider where medically appropriate.

Modifications to Terms

We reserve the right to modify these Terms at any time. Changes will be effective immediately upon posting on our website with an updated "Last Updated" date. Your continued use of our services constitutes acceptance of modified Terms. Material changes will be communicated via email or clinic notice where practical.

Severability

If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary, and the remaining provisions will remain in full force and effect.
